3. This form must be completed in Quadruplicate (Make 4 copies of this document)
and in Capital(Uppercase) Letters and submitted with Four(4) Passport Size pictures
taken in the last Five(5) working days before your intended
date of departure.
4. Full names, addresses and Telephone numbers of references
in Ghana should be stated. Refer to item 8 of the Visa Application Form
5. All applications by post should include self-addressed
stamped or registered envelopes.
